Camp forms are required by the university to participate.
1.  Camper Activity Waiver (this form can be emailed to cm69@illinois.edu or melinda3@illinois.edu in advance or brought with your camper on their first day)
2.  Camper Registration Form (this form must be filled out online)


Volunteer/Group Leader Forms
Earth, Society & Environment (ESE) Camp for Kids is looking for two types of volunteers to help run the camp, 1) activity volunteers and 2) group leaders. 
1.  Activity Volunteer Form
We need 16 activity volunteers for a variety of camp related activities. Activity volunteers will assist activity leaders with their camp activity, which may include setting up, breaking down, and cleanup. This will be a time commitment of ~5 hours. This is not a paid position and is open to all genders. You may volunteer for a specific camp activity. Volunteer spots will be on a first come first serve basis and you do not need any prior knowledge related to the camp activity. Breakfast, lunch, and snacks will be provided! To sign up as an activity volunteer please fill out the Activity Volunteer Form.
2.  Group Leader Form
The other type of volunteer is a Group Leader. We are looking for 6 Group Leaders to look after and assist a group of 5 campers each during their time at camp. The Group Leader is a paid position and is open to all graduate or undergraduate students. We welcome non-binary and genderqueer identity students to volunteer as well. This will be a time commitment of 17 hours, and you will receive $272 for your time. Breakfast, lunch, and snacks will be provided. Please Note: Campus Security Authority Training and U of I Volunteer, Contractor, and Other Designated Individuals Identification and Protection of Minors Acknowledgement forms, and background checks are required by all activity volunteers and Group Leaders as a campus wide policy for the protection of minors.
